The RPG elements are as good as people say, do the slightest thing different and it feels like a whole new playthrough.
The actual gameplay is serviceable, they actually managed to greatly improve Fallout 3's in their limited timespan, but weapons still feel pretty clunky and balance between weapons is nearly nonexistent.
Without patches or mods of any kind, this game barely runs. Probably the single worst thing about this game, but luckily also the single easiest thing to rectify.
Excellent game, any and all issues could be fixed with a remaster of any kind, and if you worry of what else Bethesda may do, you can just fix them yourself with a few mods.
