RTCW is still a great shooter, and RealRTCW is arguably the best way you can play it (even for a first playthrough) in the modern day. It feels less like a community mod and more like a Nightdive Studios-style remaster. A few options you can revert back to their old versions too.

Some of the new weapons added are great, most notably the shotgun which felt absent in the base game. It's not something I used much, but it made the usual frustrating zombie chapters much more enjoyable since it's great for dealing with them. The new pistol and assault rifle/LMG hybrid were great too.

Still do have some of the smaller issues with the game that I did before, such as the final boss being particularly bad along with the blinding ghost enemies being very frustrating. Still, this mod makes changes where it counts along with proper widescreen support. If I do replay RTCW again, this will likely be how I do it.

Short: This is the definitive way to play RTCW nowadays.

Long:
Please keep in mind it has been at least 10 years since I played the original RTCW, so it might be that I praise this mod for stuff that's already is like this in the original 2001 release.

So what first comes to mind: Everything looks better without breaking the game's original style and the lighting looks incredible.
The sound design is great aswell. The music fits and a bazooka shot flying next to your head sounds like you would imagine it.
Nearly all guns feel pretty punchy and how they should in their specific roles. Only the shotgun feels a bit weak for a shotgun, but is strong and punchy aswell and has a great range.

Downsides? There is just a handfull.
The scripted explosions are still pretty loud, even with volume turned down. So watch out, if you are sensitive to sounds like me.
The, in the later levels appearing, Super- and Ubersoldiers are horrible. They either have the hit scanning minigun, the tesla gun which has far too much range or a automatic rocket launcher, which is espacially unnerving in tighter corridors.
Additionaly they are bulletsponges. Same goes for most resurrected enemies, but in the last stages of the game it isn't a big deal anymore, as your arsenal now has more effective weapons.

The weapon and location designs in the mod look better than in the original game!
