mode tierlist:
marathon >>> classic = flash > vs. cpu > story > score attack
marathon (endless) is the best thing that ever happened to the series, and i'm surprised future games didn't capitalize on it - except dr. mario world to an extent.
story mode is a cute novelty at first, but going through hard mode without continues for the sake of unlocking vampire wario and metal mario is brutal - and super hard is a joke of a mode that the game doesn't even keep track of score for. don't do it.

I completed the story mode playing as Wario. It was a daft little story intersected with playing a battle version of Dr Mario. Whilst it was compulsive stuff, it was quite difficult but I soldiered on and thought it was ok.
I think I prefer the NES version of Dr Mario. I played that loads when the NES app was launched on Switch. There's something about this version that hasn't quite clicked. It's pretty much the same game only with added functions and jazzier graphics, but maybe I just prefer the more basic version.

I played it as a little kid way back when, and the main thing that I liked about it is that it was one of the only games my mother would play with me because she only liked puzzle games. I was bad at it, but I still think fondly of it for that reason, even if I didn't have fun with the game on its own.
It is still Dr.Mario but its got a bit more content then the rest of the series. It offers a 4 player mode which is awesome and a pretty cool story mode. There is a very large amount of playable characters which is funny. The game is still base Dr.mario so its simple and basic but still fun. Probably my favorite version of the game.
I don't think I've ever gotten as angry playing a puzzle game as I did with Dr. Mario 64.
This whole game confounds me. Why are the CPU players so frustratingly difficult to fight? What is even going on with the bizarre "plot" in the story mode? Why is it full of C-list Wario Land characters instead of more typical Mario characters? I'm just so mystified. Couldn't you have just done a roster of characters more akin to the Mario Kart or Party games of the era?
At the end of the day, Dr. Mario 64 plays fine and looks decent... but I don't know if I would ever want to go back.
