inspired by the House of the Dead franchise, specifically The Star from HOTD4 who develops bullet holes through his coat over time. specifically bosses and not fighting game characters!

Final Fantasy Mystic Quest. Every boss and enemy have their sprites get progressively deteriorated and sad-looking the lower their health is.

Whoops, just completely glossed over that cover. In that case, Doom Eternal. Enemies and the final boss have armor and flesh stripped off the more damage they take.
Banjo Tooie has a boss named Old King Coal that literally breaks apart more and more the more damage he takes
A few Xenoblade X enemies and bosses have a mechanic where you can destroy one of their limbs/appendages, both changing their appearance and at times eliminating parts of their moveset. I don't have names off of the top of my head unfortunately.
Alse the Bell Gargoyles and a few other bosses in Dark Souls who's tails you can cut off.
Dragon Ball Origins for Nintendo DS has a cute system where bosses don't have health bars but instead gradually get covered in bumps and patches the more damage you deal to them.
Also the spider boss in Cocoon loses one limb in every phase of the battle. Same goes for the spider in Limbo, if the encounter counts as a boss fight.

In Dark Souls 1 the Great Wolf Sif will start limping when at low health

The first boss in Rocket Slime is a tree that gets progressively balder as you deal damage.
Every version of Legion in Castlevania is also this, its body falling away as you knock parts off of it
