Videogames with surprising elements

Games with a sounding highlight, so no subtleties. It must be something striking and even if it's quite isolated compared to the rest of the game, it should leave a mark. Should be something integral to the core/aesthetic of the game(play), so I wouldn't put something like the dog ending of Silent Hill 2.

How the moon looks like.
- The bullfighting part were you kill cows.
- Shit/poo presence.
Its pure cheap appearance, and antics.
(To be fair, I haven't played a lot of this game, but if you're not agree with this perception, I'll see what you think in the comments.)
The increasing length of the battles.
A black background that works. Makes you feel isolated.
The fact that you're more probably going to kill you than anybody else.
(This may be applied to the rest of the series, but it was important to transfer that feel to a 3D space)
(H game)
Everything is decided randomly using a roulette. And the combat system is based in luck too, with a dice whose number is linked to the actions you assigned to each member of the party.
A game designed to build an approachable hectic rythm.
I don't think there's a "narrator" like Bennett Foddy.
- Parameters in the middle of the screen.
- Dreamlike imagery and setting based on the sea and coasts.
The OST/Sound design
There may be more, but this is a slow ninja game. At least compared to Ninja Gaiden.
An expanded approach of commands interacting with (possible) foes.
[Edit.: I've the feeling that this description will fall short compared to a lot of (C)RPGs. The thing is that the action command that plays like a shmup where you have to avoid bullets is an original adition.]
This is a "negative" highlight, because it's a small but stricking element that makes me dislike the game a bit:

The harshness of the deaths. Specially when it happens to others, like the cavemen.
