Loosely based on the film Gotcha!
There are two games based on Indiana Jones and the Last Crusade on the NES with the same exact title and nearly identical cover art, but both are completely different games. One is developed by Taito, and the other is developed by NMS Software.

Based on the film Puss 'N Boots Travels Around the World
Loosely based on the film Wheels on Meals
Very loosely based on the film Aces: Iron Eagle III

Side note: I made this list because I was planning on going through all of these and ranking them. Its been a few months now and I kind of just forgot and now I don't care to subject myself to even more garbage. New Ghostbusters II is pretty good though!
Spartan X is so funny. Imagine being so inspired by one of the best action comedies made that you inadvertently laid the foundation for the action and BEU genres.

@BlazingWaters I still need to watch Meals on Wheels. In fact, most of Jackie Chan's filmography I still need to watch!

@Ratts Swamp Thing seems to be based on the animated series, not the film. The other three have been added, thanks!

Not sure if it counts but there's a game called Toki no Tabibito: Time Stranger by Kemco based off the Anime film of the same name. Though it's infamous for not even following the source a single bit, the girl on the poster isn't even in the game.
