Mostly going from the workshop in steam as it is my primary platform for modding. I'll add games and try to put in the notes some notable mods. Please sugest games and note some mods!
Total conversions and heavy gameplay changes take priority.

Rivals of Aether: tons of homemade characters ranging from OCs to George Constanza

Terraria: waves and waves of mods adding items, mechanics, and even changing the game completely such as Thorium and (infamously for some) Calamity

Probably an extension since Half Life is already here, but Half Life 2 -
Back in the day was a treasure trove of standalone single/multiplayer mods for Source like Neotokyo, Dystopia, Synergy, Zombie Panic, Gmod, Minerva Metastasis, Nightmare House 2 and Research and Development

I've been really loving the Celeste modding community, with the Spring Collab being the most impressive thing I've seen. Also I don't know if hacks count as modding for the sake of this list, but Super Mario World seems like a must-shout-out too if so

A Link to the Past: one of the OG randomizer mods, still updated with a lot of different modes and a very active community. Also notable is the ALttP/Super Metroid cross-game randomizer, which is a very impressive mod. Super Metroid itself has a lot of romhacks made over the years as well.

Super Mario World: responsible for popularizing romhacking and "kaizo" mods/games, many impressive mods made in it over the years, list of some examples here

Super Mario 64: has a ton of romhacks, in addition to the fan PC port if you count that as a mod. List here.
