Lev is a trans boy. He had been kicked out of his religious community known as the Seraphites for shaving his head and identifying as a man. During a confrontation with some Seraphites, they angrily refer to Lev by his deadname.

Janeva's gender is not stated, though they assert that they are "not one of [Aloy's] sisters" and are clearly a soldier for Carja, a profession exclusively for men.

Bloodhound is nonbinary and Catalyst is a trans woman.

The English version of the game and social media accounts avoid using gendered pronouns when referring to Blanche. (The Spanish, French, Italian, and Brazilian Portuguese translations refer to them as female. The German translations referred to them as female as well, but German social media accounts stopped using gendered pronouns for Blanche after 2020.)

Naoto is initially introduced as a man and presents as such, wearing the male school uniform and purposefully deepening his voice. When you get to his dungeon, his Shadow reveals that he was not assigned male at birth, and is actually a woman who disguised himself as a man to avoid misogyny in his line of work, going as far as to desire sex reassignment surgery. His dungeon also features a lot of medical themes.

The scene that takes place the next day when Naoto returns to school and joins the party features NPCs gossiping about Naoto's gender, similarly to an outing of a trans person. Naoto is shown very uncomfortable with this.

After this reveal, Naoto continues to bind their chest and wear more masculine clothing, but should the player want to unlock Naoto's romance route, the protagonist has to tell Naoto to be more feminine. During an intimate moment between the protagonist and Naoto where he expresses his wish to have been born a boy, you are able to initiate the romance route by responding, "I'm glad you're a girl."
Later, after confessing your love, you are able to ask him to change the sound of his voice in a higher pitch to sound more feminine. If you do so, Naoto comes to school in a girl's uniform during the Christmas event, despite being very visibly uncomfortable and quickly asking to change back into his masculine clothes.

Naoto also expresses discomfort in a hot springs scene where his naked body is paired alongside his cis female companions.

Shigenori Soejima, the art and character designer of Persona, says that Naoto was intended to be male all along, but that would have made for a boring story that lacks the conflict of the canon story.

An official Line Sticker also shows that Naoto binds his chest with bandages.

Also take into consideration that all of Naoto's Personas are male mythical figures.

Chihiro is initially introduced as a female student and presents as such.

In Chapter 2, Monokuma threatens to reveal a "secret" that each of the individual characters have. Because of this, Chihiro attempts to build trust by telling her secret to Mondo, a person she admires, which is that she was born male and dresses up as a girl due to bullying she faced as a child for "not being manly enough" and "being so weak for a boy". Chihiro then asks Mondo to help her become stronger, but this only lead to Mondo being overcome by rage and jealousy at Chihiro’s strength and courage, feeling humiliated and "unmanly", and as a result, he strikes Chihiro over the head with a dumbbell, killing her.

Chihiro's assigned gender is outed to the other characters during the following trial.

Giovanna's bestiary description reads as follows: "Giovanna resented being assigned Mage at birth, and proudly took up the way of the broom and pointed hat. Her sisters, who claim they want to 'protect witches' spaces,' have sworn to oppose her and all who fight by her side. Maybe one day they'll accept that anybody can be a Sassy Witch if they truly want to be."

She is a Beauty, an exclusively female trainer class, who states that she used to be a Black Belt, an exclusively male trainer class. In the Japanese version of the game, she explicitly credits her transformation to medical science, but this was cut from the English version.

Clove is nonbinary and uses they/them pronouns.

Liara T'Soni belongs to a single gendered race called the Asari, a species that has traditionally feminine traits but reproduces by means other than sexual reproduction. Liara can also state that "male" and "female" have no real meaning to Asari when female Shepard asks if she would like to enter a relationship despite them both being women.

The city councilwoman of San Francisco, Miranda Comay, is a trans woman. She is a longtime friend of the protagonist Marcus and his family, and helps Dedsec in their mission to take down the central Operating System and the corrupt company behind it.
At one point in the game, she mentions that the religious group Church of the New Dawn hacked her medical records and released footage of her sex reassignment surgery to the public.

In the first-edition manual for the North American release of the game, Birdo is referred to as a "male who believes that he is a female" and would rather be called "Birdetta", making her the first transgender character for Nintendo.
In the Japanese version, Birdo is referred as "Catherine" where it is also stated that the character is a man who wishes to be a woman. In later printings, mention of Birdo being male was omitted, and further not included in most later games with the character.

Arcsys confirmed Testament was never intended to be male and always went by they/them pronouns.

Bridget's arcade mode story revolves around her being unsure whether she is truly happy as a boy and is being her true self, which is revealed to be her questioning her gender identity in one of the endings. Depending on how well the player performs, she makes progress towards figuring it out and accepting her new identity, with one of the endings directy showing her coming out to Goldlewis Dickinson and Ky Kiske, proudly stating that she is a girl.

Other characters, in-game bios, and official books refer to Mangle with conflicting gendered pronouns and terms. In this game, Phone Guy refers to Mangle as male, but in the custom night "Ladies Night," Mangle is included with Toy Chica and Withered Chica.
In 2015, Scott Cawthon even addressed the question of Mangle's gender by (albeit jokingly) stating "Is Mangle a boy or a girl? The answer is yes."

The Puppet is also referred to with either they/them or it/its.

Scott Benson clarified on CuriousCat that he had intended for this to come up at some point in the first version of the game, but the information had gotten lost in the shuffle as scenes were rewritten and trimmed, and he didn't realize it was still missing from the final cut until it was too late. As of Weird Autumn edition, Bea now mentions to Mae that the name of Jackie's single-person band was "Trans World Order Give Them Hell"

The symbol on Mae's shirt is also a common nonbinary symbol, though Scott Benson was unaware of this and had not intended for Mae to be nonbinary.

Poison is introduced in Final Fight (1989), where she is either a pre-op or post-op trans woman, depending on the region where the game was released.

She is an antagonistic NPC in the arcade beat 'em up games Final Fight (1989) and Mighty Final Fight (1993). She is a playable character in the fighting games Final Fight Revenge (1999), Street Fighter X Tekken (2012), Ultra Street Fighter IV (2014) & Street Fighter V: Arcade Edition (2018).

In Street Fighter V: Arcade Edition, her identity is alluded to more often. Certain dialogue during a conversation with Lucia alludes to the fact that she was born with a gender not matching with her inner being. One of Poison's win quotes, "Being gossiped about is proof of my popularity!" seemingly alludes to the controversy and ambiguity surrounding her gender identity.

When the game initially released, Hainly Abrams would readily inform the player that she is transgender and reveal the name she was given at birth. She tells the player that she left the Milky Way galaxy searching for greater acceptance of her gender identity. The game was later patched so that she would only reveal this information if the player developed a supportive relationship with her.

Paolo is a trans man. He was once in the Fuerzas Nacionales de Defensa but was expelled after his father, a drill instructor, found out about his identity. According to Matías Alonso, Paolo underwent top surgery without any anesthetic and barely flinched.

Ubisoft themselves have also described Paolo with the following: "He's trans in a country that refuses to accept him for who he is."

An argonian NPC named Chal-Maht is a trans man, stating he used to be born female before changing to a man through the Hist. Similarly, Kud-Nakal, a friend and love interest of Chal-Maht's, is a trans woman who also changed to female through the Hist.

He explains that in the Japanese version, any pronouns are not used to refer to Marco, but since it is difficult to construct sentences without pronouns in English and because players might be confused* if "they" or "ze" suddenly appeared in dialogue, the localization team decided to use he/him.

A substory involves an NPC named Ayaka, who is a trans woman. Talking to her at the Bantam bar will involve her asking for Kiryu's advice on how to ask out a man she is romantically interested in, eventually confessing to Kiryu about her birth assignment. She goes on into explaining her transition and how she underwent gender reassignment surgery, which is why she was unsure whether to ask out to the guy she likes since she worries about how he'll react. Kiryu is initially surprised by this, and you have the option of saying it's a bit difficult to come to terms with, but he is overall supportive of Ayaka, telling her while he can't say how the guy will react, if it goes badly it's not her fault.

At some point after the quest "The Light Within" (released in 2015), the player can show Angof a wedding ring, prompting her to tell the player she was born male, but "corrected" herself to female once she could shapeshift.

There is an entry in her diary that also reads:

This is a LOT better than my list which I gave up making not even halfway through. Thank you, for making this!
You mentioned in the description that you are open to suggestions so here's my minor input.
- Cyberpunk 2077's description says that you can create a custom trans character, however, there's also a fully voice acted trans character called Claire, who gives you the racing side-quests that might be notable.
- Hogwarts Legacy also has a trans side character, the bartender called Sirona Ryan in Hogsmeade. It also lets you create your custom character as trans.
- It's not out yet but I've been paying attention to Goodbye Volcano High, which according to their FAQ has a non-binary main character called Fang. Source: https://goodbyevolcanohigh.com/faq/
- The Sims 4 and Baldur's Gate 3 both let you create trans custom characters.

I actually was aware of the character in Hogwarts Legacy, but given JKR's history of transphobia and the fact that she considers monetary support of the HP series to be support of her bigoted views, I actually decided against including the game at all. I know she wasnt directly involved with the creation of the game, but I still felt giving the game any attention would still benefit her. Hope this makes sense, thank you again for your suggestions!

Hi, again! Thank you for including my previous suggestions, I hope you don't mind some more.
- The Comic Wheel Sisterhood has a character who is struggling with dysphoria and the main character helps her out. You help her choose her name from a set of choices, so I don't know what to call her canonically, but I'm pretty sure she's referred to as The Peppermancer.
- Starfield lets you choose any Body Type, Walk Animation, and even pronouns including they/them.
Spoiler?: There's also a female character called Hadrian who is originally a "clone" of a man. I'm not sure if she classifies, but I did see a bald white dude having an embarrassing meltdown over it on Twitter, so I figured it might be notable.

I promise I won't spam you with these. It's your list after all. I also understand and respect your reasoning about excluding Hogwarts Legacy. I hope I didn't come off as one of those people by suggesting it.

Adding to Mama from Somnium Files, an exchange in the second game suggests she has had bottom surgery. 7th Dragon 2020, at least in the English fan translation, features a sidequest from a "transfixing woman" to retreive her "blue pill bottle", which she says contains hormones that are very important to her. Additionally, an NPC in the sequel, 7th Dragon 2020-II, will greet the party with "Mr, Ms, Mx Murakumo!", implying that at least one member of the player-created party could be non-binary.

I have a couple of suggestions if that's okay, sorry if they're already present and I didn't see them. In Stray Gods: The Roleplaying Musical, the latest incarnation of Hermes is nonbinary. In Harmony: The Fall of Reverie, Nora is a trans woman and Yana, Chaos, and Truth are nonbinary.

For a suggestion I didn't see on here, The Arcana allows you to be nonbinary, one of the romance options (Asra) is nonbinary and uses he/him pronouns, and several major side characters (Valdemar, Nazali, and Vulgora) are nonbinary with they/them pronouns. There is also a minor character, Scout, who mirrors whatever pronouns your character uses, which can include they/them.

I see you already have Heart of The House by them on here, and I have no idea how you'd feasibly list them because there are so many games, but if you're not aware having nonbinary/trans options and diverse characters aside from yours are the norm for any interactive fiction game published by Choice of Games or Hosted Games! Off the top of my head a few notable ones are:

- Creme de la Creme (protagonist can be nonbinary, all romance options can have their genders selectable or randomised which both include nonbinary)
- Fallen Hero: Rebirth, Fallen Hero: Retribution (protagonist can be nonbinary; one of the romance options (Dr. Mortum) is trans, whether a trans man or a trans woman or intersex will depend on what gender you selected them to be; a major side character, Hollow Ground, also does the same as Scout from The Arcana in that they mirror whatever pronouns your character uses)
- Keeper of the Sun and Moon, Keeper of the Day and Night (protagonist can be nonbinary)
- The Wayhaven Chronicles (protagonist can be nonbinary)
- Heroes of Myth (protagonist can be nonbinary; Min, one of the romance options and a major character, is nonbinary and uses they/them)
- Versus (protagonist can be nonbinary; Prince Everly, a romance option, is nonbinary with they/them pronouns; Breeze, another romance option, is either a trans man, trans woman, or intersex depending on the gender you select for them; Oli, another romance option, at first appears to be whatever gender you selected but turns out to be the false identity for a character named Dhanthik, who is nonbinary and uses zie/zir pronouns).
